Complaints, Returns Forms

Customers who intend to submit a remedy for a defective product or are going to apply for a refund of a purchased product have three types of forms to choose from:

• The WORD interactive form - was prepared in Microsoft WORD and the user should open it in the 2010 version or higher.
• The standard WORD form - was prepared in Microsoft WORD and the user should open it in the 2010 version or higher.
• A standard PDF form - it has been prepared in Adobe Acroba Reader and the user should open it in any program that allows opening pdf documents.

If the forms are sent electronically, they are not required to be signed.
